The Center for Respite Care is a 14-bed 24-hour facility for homeless people who are either too sick for a shelter or have been recently discharged from a hospital and need a safe place for recuperation. These patients receive basic short-term medical and nursing care as well as social services while they recover from illnesses.
Clients must meet the following criteria for admission:

Client MUST be referred by a medical professional - NO exceptions

Must be homeless (HRSA compliance, includes double-ups)

Must be an adult (18 years of age or older)

Must have a medical referral, will acute illness or injury as primary diagnosis

Must be able to perform activities of daily living, or ADLs (self care)

Must be ambulatory

Must be able to follow medication guidelines

Must have a stable mental status
At the Center for Respite Care, clients will receive:

Medical evaluation by medical doctor or nurse practitioner

Nursing care
-
Lab testing
-
Semi-private room
-
Three meals per day
-
Referrals to social services, housing agencies or shelters
Clients must have a medical referral, from a medical professional, from one of the following sources:

Hospital

Medical Van (the Med Van)

Health Resource Center

Health Clinic

Referrals will be reviewed by doctor and intake nurse
